Premiums are calculated by $0.115 per $1000 of coverage.
Long Term Disability - Madison National Life
Coverage is for employees only.
Premium share and initial eligibility is determined by your employment contract. Please refer to your contract for further information.
Premiums are calculated by $0.245 per $100 of the employee’s annual salary.
Benefit is 66 2/3% of employee’s annual salary after a 90 day calendar waiting period.

Supplemental Long Term Disability (in addition to Long Term Disability) - Madison National Life
Supplemental benefit is in addition to the basic Long Term Disability benefit.
Enrollment is not guaranteed - enrollment is granted at the discretion of Madison National Life.
Supplemental premiums are calculated by $0.21 per $100 of the employee’s annual salary.
Benefit is 85% of the employee’s annual salary after a 90 day calendar waiting period.

After-tax policies or changes are allowed at anytime.
